They’ll connect you with an intake coordinator to help determine your child’s eligibility for different services. Once approved, your family is assigned a service coordinator to discuss your child’s options in more detail and guide you through the next steps.
At Harmony House, we require everyone interested in our day programs or Independent Supported Living (ISL) to schedule a tour. Visiting our spaces gives everyone the freedom to choose where they feel most at home, starting their journey toward confidence with an independent, but supported, decision.
If your loved one is interested in joining our full-time day program or Independent Living Services (ISL) group homes, you must be approved for a Comprehensive Waiver. We require a Community Waiver for part-time day services, and a Hope Waiver for limited day services.
Usually, approval takes about 6 weeks, but timelines may differ if additional documentation is required. After determining your loved ones’ eligibility and identifying the appropriate services, your service coordinator submits a funding request to the Utilization Review Committee.
If your service coordinator hasn’t returned your calls or responded to your emails, reach out directly to their supervisor for support. You deserve prompt guidance, and remember that guardians and individuals can always request a new service coordinator.
